Completing the Order Form
  1. Photocopy the STS order form that corresponds to the test you would like to order. You must enclose a purchase order or prepayment (plus 15% for shipping and handling or actual billing for purchase order use). Sample Set prices include postage for all U.S. shipments.
  2. Completely fill out “Ship To” and “Bill To” on order form. NOTE: United Parcel Service (UPS) cannot ship to a P.O. Box. Please use complete street address.
  3. If the purchase is to be charged on VISA®, MasterCard®, or American Express®, completely fill in the appropriate area under “Ship To” and “Bill To.” Please include the name, address, and phone number of the cardholder.
  4. Fill in the testing and approximate arrival dates in the “TESTING INFORMATION” section
  5. Carefully identify the materials you are ordering. Specify the exact title, grade level, form, and quantity in the same terms as the catalog listing.
  6. Carefully indicate the scoring method to be used—in the same terms as the catalog listing.
  7. Send your order to Scholastic Testing Service, Inc., 480 Meyer Road, Bensenville, Illinois 60106–1617.
  8. If you are in the continental United States and have a purchase order, VISA®, MasterCard®, or American Express®, call STS toll free at 1.800.642.6STS (6787).
  9. Outside the continental United States, call 1.630.766.7150 or fax 1.630.766.8054 (see No. 3 under “Shipping”).
